Home Schooled Students who want to take courses at SCC
HOME SCHOOL STUDENTS
Home schooled students have the opportunity to attend classes at SCC while completing their requirements to complete high school. This program runs out of the Early Outreach Programs office.

ALL TRANSACTIONS REQUIRE PICTURE ID
Paperwork must be completed EVERY semester
Concurrent Enrollment/Special Admissions Form (see below for the downloadable document):
1. PRINT DOCUMENTS AND COMPLETE FORM PER INSTRUCTIONS:
2. REQUIRED - Parent/Guardian Signature
3. REQUIRED - School Official Signature - Parent can sign since they are officially
the "school".
4. REQUIRED - HOME SCHOOL AFFIDAVIT OF INTENT from the Maricopa County
School Superintendent, if student is less than 16 years of age. For
information on the Affidavit of Intent call (602)506-3144 or get the form
by clicking on Home School Affidavit of Intent.
5. REQUIRED - If student is under 16 years of age, college instructor's signature
is required to enroll in the course.
6. REQUIRED - Unofficial Transcript - If you have a transcript from Home Schooling,
please bring it with you to your appointment.
7. REQUIRED - Take placement tests - Reading, English and Math to determine
placement into courses.
8. REQUIRED - Bring proof of citizenship/legal status; Driver's license, Driver's
Permit, Birth Certificate, Passport or Tribal ID if applicable. (REQUIRED
ONLY ONCE)

SUBMIT COMPLETE FORMS, in person, to SC-155 located on the SE corner of the Student Center Building.

NO PICTURE ID? Contact the Department of Motor Vehicles. If the student has a picture ID from a previous high school, this will work for in-person activities at the college. SCC requires students to have a picture ID for all transactions.
READ IMPORTANT INFORMATION
To determine placement in a course, Home School students are required to take the "block test" for students new to college. If you have a transcript from a previous school, please bring a copy.
If a parent is registering a student - Written AUTHORIZATION from the student is REQUIRED. It must be signed and dated by the student. Parent must have picture ID.
